Food Box Distribution
Every Wednesday, we have complementary food box distribution. Registration/Check in begins at 9:30am and ends at 11:45 am. Food box shopping is from 10 am until 12 noon.
All clients must call in to 602-264-0223 or email to firstname.lastname@example.org by 5PM on the Sunday prior to the distribution day in order to receive a food box.
If this is your first time coming to Joshua Tree, you will need the following:
- Client Application
Click Here for the application. If possible, print, complete and bring with you. If you do not have a printer, this document can be provided to you upon your arrival, however this will lengthen your registration process.
- Client Contract
Click Here for Client Contract and Rights & Responsibilities. This document is 23 pages long. There are 2 pages that you must be sure to print which are pages 19 & 23. Page 19 is a Release Of Information and Page 23 is acknowledgments that you have received and understand the Client Contract, Grievance Policy and Privacy Practices. These 2 pages must be completed and returned with your application. If you do not have a printer, this document can be provided to you upon your arrival, however this will lengthen your registration process.
- Proof of Residency
Example: Government Award Letter, Lease, Utility Bill, Driver's License, etc...
- Proof of Income
Example: Government Award letter, 1 month of pay stubs, etc...
- Proof of HIV Status
Example: HIV Drug Prescription, positive HIV test results or detectable viral load blood test, Letter from MD
You will be required to update these documents on a yearly basis. Changes to your residence and income should be reported immediately.
